Definitive Subdivision Plan (Form C)
Consultant Review Fee* $ 2,500.00

Major Site Plan with Hearing
Construction Phase (due upon project approval) $ 2,000.00

Assisted Living (ALF) and Independent Living (ILF)
Facilities Special Permit (Article 11)
Construction Inspection Phase* 3-20 units $ 6,000.00
More than 20 units $ 6,000.00 base fee
Plus for each additional unit over 20 units $ 300.00

Common Driveway Special Permit (Article 12)
Application $ 125.00
Consultant Review Fee* $ 500.00

Flexible Frontage Special Permit
Application $ 125.00
Consultant Review Fee* (Additional deposits may be required) $ 800.00
Advertising/Abutter Notification $ Actual Cost

Noquochoke Overlay District Special Permit (Article 19)
Application $ 500.00
Consultant Review Fee* $ 2,000.00
